Comparison of pressures measured from the proximal external jugular vein and from a central vein.

چکیده
Sir,—The study by Shah, Swai and Latto (1986) comparing the venous pressures measured from the proximal external jugular vein and from a central vein was interesting. The authors found a mean difference in pressure of 3 mm Hg from the two sites which was not influenced by alterations in the head position or the side of jugular venous catheterization. Whether the reference zero is the right atrium or manubriosternal joint, the manometer scale or transducer must bear a constant relation to the zero (Lawlcr, 1980). The scale or transducer should be fixed to the patient or the operating table, rather than to a freestanding pole. The authors have failed to mention the position of the transducer in relation to the zero point. As the measurements were taken in the horizontal position, one is compelled to believe that the transducer was at the body level. Although a source of error did exist, it was probably equal for all the measurements. As long as the body position is horizontal, the measurement from a slightly distant vein like proximal external jugular may reflect near true venous pressures, as the authors have found. This is not necessarily true if the patient is tilted or slightly head-up on the operating table or intensive care bed. Then the position of the transducer becomes more important. Moreover, changes in the central venous pressure arc more important than a single value, particularly after fluid challenge. It remains to be seen what the effects of posture and fluid challenge would be on the pressures measured from the proximal external jugular vein and a central vein.

Facial Vein Draining Into External Jugular vein (Case Report)
Purpose: Circulatory system is very important because of injection, Blood transfer and centeral venous catheter insertion. The superficial veins, especially the external jugular vein (EJV) are increasingly being utilized for cannulation to conduct diagnostic procedures or intravenous therapies. The anomalous drainage of veins is of great significance for vascular surgeons and reconstructive sur...
متن کاملThe efficacy of inferior vena cava diameters and the jugular vein in assessing fluid resuscitation
Objective: Fluid resuscitation is necessary in almost all critical patients. The central venous pressure (CVP) is a well-established method of assessing resuscitation. Recently, there have been attempts to investigate less invasive methods like the diameters of inferior vena cava (IVC) or the jugular vein. We aimed to investigate this method in our research. Methods: Seventy eight critical...
متن کاملA Case Report: Variation of the Cephalic and External Jugular Veins
The knowledge of variations in the superficial veins is of clinical importance for the anatomist, radiologists, clinical practitioners and surgeons in order to plan about the operative procedures. Usually cephalic vein drains into the axillary vein. In this case report study the left cephalic vein communicates with the left external jugular vein and made a common trunk at th...
متن کاملA comparison of external and internal jugular venous pressures to monitor pulmonary artery pressure after superior cavopulmonary anastomosis.
The internal jugular vein continues to be the preferred site for cannulation to monitor central venous pressure despite the reported evidence of the accuracy of external jugular venous pressure (EJVP) to reliably predict internal jugular venous pressure (IJVP). Internal jugular venous cannulation carries a risk of thrombosis that can be life-threatening in children undergoing superior cavopulmo...
متن کاملA comparison of jugular and central venous pressure measurements during anaesthesia.
Simultaneous measurements of the jugular and right atrial venous pressures were made in 16 patients undergoing cardiac and vascular surgery to see whether the jugular veins were a reliable guide to central venous pressure. It was found that with the chest closed both the external and internal jugular veins on the right side provided a good guide. The vessels on the left side were less consisten...
